      Occupational Burnout Symptoms and Its Relationship With Workload and Fear of the SARS-CoV-2 Pandemic Among Hospital Nurses

          Abstract

          Introduction

          The pandemic has intensified physical and psychological work demands experienced by nurses in a hospital environment. The purpose of this study was to examine personal and work environmental risk factors associated with occupational burnout among hospital nurses.

          Methods

          We conducted a cross-sectional from April to November 2020. Data from 831 nurses who worked professionally in four educational hospitals were compiled through survey questionnaires to report the prevalence of burnout, occupational and individual factors. Independent t-test and Mann–Whitney test measured the link between the scopes of occupational burnout and risk factors.

          Results

          About half of the participants indicated moderate symptoms of burnout. The fear of the nurses correlated significantly with emotional exhaustion ( r = 0.71, p = 0.001), depersonalization ( r = 0.67, p = 0.02), and personal accomplishment ( r = 0.63, p = 0.05). Mental demand ( r = 0.74, p = 0.01) and effort at work ( r = 0.68, 0.001) correlated significantly with emotional exhaustion ( r = 0.51, p = 0.03).

          Conclusion

          The findings indicated a high prevalence of burnout symptoms, particularly emotional exhaustion, among hospital nursing professionals. Occupational health services should consider burnout as an occupational-related condition and provide interventions to reduce workplace chronic stressors and burnout in hospitals.

          Work stress among Chinese nurses to support Wuhan in fighting against COVID‐19 epidemic

          Abstract Aims To investigate the work stress among Chinese nurses who are supporting Wuhan in fighting against Coronavirus Disease 2019 (COVID‐19) infection and to explore the relevant influencing factors. Background The COVID‐19 epidemic has posed a major threat to public health. Nurses have always played an important role in infection prevention, infection control, isolation, containment and public health. However, available data on the work stress among these nurses are limited. Methods A cross‐sectional survey. An online questionnaire was completed by 180 anti‐epidemic nurses from Guangxi. Data collection tools, including the Chinese version of the Stress Overload Scale (SOS) and the Self‐rating Anxiety Scale (SAS), were used. Descriptive single factor correlation and multiple regression analyses were used in exploring the related influencing factors. Results The SOS (39.91 ± 12.92) and SAS (32.19 ± 7.56) scores of this nurse group were positively correlated (r = 0.676, p < .05). Multiple regression analysis showed that only children, working hours per week and anxiety were the main factors affecting nurse stress (p = .000, .048, .000, respectively). Conclusions Nurses who fight against COVID‐19 were generally under pressure. Implications for Nursing Management Nurse leaders should pay attention to the work stress and the influencing factors of the nurses who are fighting against COVID‐19 infection, and offer solutions to retain mental health among these nurses.

            Global prevalence of burnout symptoms among nurses: A systematic review and meta-analysis

            WHO recently declared burnout as a "occupational phenomenon" in the International Classification of Diseases 11th revision (ICD-11), recognizing burnout as a serious health issue. Amongst healthcare workers, nurses are known to struggle with burnout symptoms the most, carrying serious consequences for patients, other healthcare professionals and healthcare organisations. Evidence has suggested that burnout symptoms in nurses is high across specialties and countries, but no meta-analysis have been performed to investigate burnout symptoms prevalence in nurses globally. We conducted a systematic review and meta-analysis to examine burnout symptoms prevalence in nurses worldwide using 8 academic research databases. Risk of bias, heterogeneity and subgroup analyses were further conducted in the meta-analysis. 113 studies were included for systematic review and 61 studies for the meta-analysis, consisting 45,539 nurses worldwide in 49 countries across multiple specialties. An overall pooled-prevalence of burnout symptoms among global nurses was 11.23%. Significant differences were noted between geographical regions, specialties and type of burnout measurement used. Sub-Saharan African region had the highest burnout symptoms prevalence rate while Europe and Central Asia region had the lowest. Paediatric nurses had the highest burnout symptoms prevalence rates among all specialties while Geriatric care nurses had the lowest. This study is the first study to synthesize published studies and to estimate pooled-prevalence of burnout symptoms among nurses globally. The findings suggest that nurses have high burnout symptoms prevalence warranting attention and implementation. This study serves as an impetus for intervention studies and policy change to improve nurses' work conditions and overall healthcare quality.

              Determinants of burnout and other aspects of psychological well-being in healthcare workers during the Covid-19 pandemic: A multinational cross-sectional study

              The Covid-19 pandemic has placed unprecedented pressure on healthcare systems and workers around the world. Such pressures may impact on working conditions, psychological wellbeing and perception of safety. In spite of this, no study has assessed the relationship between safety attitudes and psychological outcomes. Moreover, only limited studies have examined the relationship between personal characteristics and psychological outcomes during Covid-19. From 22nd March 2020 to 18th June 2020, healthcare workers from the United Kingdom, Poland, and Singapore were invited to participate using a self-administered questionnaire comprising the Safety Attitudes Questionnaire (SAQ), Oldenburg Burnout Inventory (OLBI) and Hospital Anxiety and Depression Scale (HADS) to evaluate safety culture, burnout and anxiety/depression. Multivariate logistic regression was used to determine predictors of burnout, anxiety and depression. Of 3,537 healthcare workers who participated in the study, 2,364 (67%) screened positive for burnout, 701 (20%) for anxiety, and 389 (11%) for depression. Significant predictors of burnout included patient-facing roles: doctor (OR 2.10; 95% CI 1.49–2.95), nurse (OR 1.38; 95% CI 1.04–1.84), and ‘other clinical’ (OR 2.02; 95% CI 1.45–2.82); being redeployed (OR 1.27; 95% CI 1.02–1.58), bottom quartile SAQ score (OR 2.43; 95% CI 1.98–2.99), anxiety (OR 4.87; 95% CI 3.92–6.06) and depression (OR 4.06; 95% CI 3.04–5.42). Significant factors inversely correlated with burnout included being tested for SARS-CoV-2 (OR 0.64; 95% CI 0.51–0.82) and top quartile SAQ score (OR 0.30; 95% CI 0.22–0.40). Significant factors associated with anxiety and depression, included burnout, gender, safety attitudes and job role. Our findings demonstrate a significant burden of burnout, anxiety, and depression amongst healthcare workers. A strong association was seen between SARS-CoV-2 testing, safety attitudes, gender, job role, redeployment and psychological state. These findings highlight the importance of targeted support services for at risk groups and proactive SARS-CoV-2 testing of healthcare workers.
